You could also adjust smtp_accept_queue_per_connection in your exim.conf file. The default is 10, which is way too low when you're running fetchmail. If, for instance, you've been offline for a while and then fetchmail grabs a whole bunch of new messages, the first 10 will be delivered and the rest will sit in the queue for a while.
