I've tried this now and I don't think it works; sorry. If you find a solution please post it. 

This does work, I think:

-dDEVICEWIDTHPOINTS=w -dDEVICEHEIGHTPOINTS=h


I'm a little confused about the units of w & h, but at least it gives some control and overrides the page size.
