I have a shuttle, a mac mini, and another that has a VIA Nehemiah and
is entirely fanless (the shuttle and the mac minis both have fans, but
pretty decently quiet ones).  They are mostly great, although there are
some heat issues from being crammed in a small case.  Mac minis also
have pretty bad disk I/O, due to having laptop drives.  The shuttles
have a slightly wonky CPU fan that rattles loose in a year or two and
ceases to be as quiet, although it's still not as loud as a normal PC.
The shuttle power supplies also tend to blow a little prematurely, due to
overheating and the fan bearing seizing (2 for 2 so far).  I suggest an
external power supply for these sorts of small quiet machines - the mac
mini and the VIA both have one, and they show no signs of having issues.

If you're not looking for speed daemons, then they Just Work(TM).  I am
a big fan of the small and quiet computer (probably because I have a
bunch running in my office all the time).
