You can also use Xournal  or similar programs designed for tablet computers.

They will read in a PDF and let it act as the 'background', then you can write into an "overlay" with either characters or stylus input, and save the resulting PDF with both
layers.

The date entry is free-form, however, and you can write anywhere, not just the fields.

I use the to receive faxes (as pdf) markup on the tablet , and re-fax them out electronically
while traveling to avoid need for printer and fax machine.
