It's a SuperMicro YM-5201D.  Best I can tell is that it's just a
little longer than a FlexATX size (193mm vs. 150mm).  It needs to have
a 8" 20- or 24-pin connector, a 4" molex connector, and either a 16"
SATA or 24" molex connector.  Thanks for your help, guys.
