If you want to go high-end try out the Avocent CPS-1610 + DSView software... excellent equipment (but at a price).

As far as BIOS, we use Tyan S882D boards and they support BIOS-over-serial console.

When I made some serial null modem cables, I used regular network cables
and added DB-9 adapters.  That way I could easily change the length of
