I used skype, with a stand alone netgear skype phone for about a year,
and was very disappointed.  I know you aren't asking about skype, but
just a thought.
I now use T-Mobile's voip option and have been very happy.  For
$10/mo, we get unlimited long distance (US only) on a home line.  You
can use any standard land-line phone with the Cisco router from
T-Mobile.  Then for $5 more per cell phone we have, the phones also
have voip and unlimited calling when using voip.  Consequently, my
wife and I only use about 100 of our 700 minutes combined per month,
always outside of the house calling.  Just a thought...
