This is not a script that you install, it is a complete service, including secure server, for $30 per month. Given your requirement for something simple, this is as simple as it gets: no installation, no messing with ssl certificates, just code a form for each item in your HTML and you are live. If you want, you can upload your items into their database and they will generate the HTML for you. Not free, but very flexible, good, and easy. (I do not receive any financial considerations from Americart ).
