[KSB] A simpler solution: create a set of text documents in OpenOffice.org with the PDF pages (may need to convert formats to jpg or eps) one for each page and put the fax images as background.  Then put a frame with text wherever you want to add comments.  You can re-export as a PDF if you want.  Pretty straightforward stuff.  You can even annotate pictures this way.
