I've heard good things about voicepulse.
http://connect.voicepulse.com/  They will do IAX/SIP origination or
termination.  It is basically a pay for what you use, so you could set
up an asterisk server and your own voicemail box and then forward
calls around as you see fit.

They have other plans to geared toward residential and business.
