I use SIPTalk.  Good web control panel.  Easy to setup VM, Call Forwarding, etc.  I also like their Call Blast feature.  Multiple phones ringing on the same number dialed.

Wide enough variety of rate plans and cheap international rates.  My 2 hour weekly conference calls to Sweden cost less than $3.
