Most home routers support UPNP for exactly this type of application
these days. There's some perl and python libs that let you control
upnp devices. I haven't used any of these myself, but IM clients like
pidgin and p2p clients like azuerus use it to open ports in your
router when needed.

You could probably script something that would launch vnc and setup
the upnp port mapping.
