If I had to setup a good NTP server I'd consider running nanoBSD on an
unmodified Soekris NET4501.

If I was needed an awesome one, I'd even consider trying the hardware
mods described here.

Apparently, the crappy old AMD Elan520 CPU in the NET4501 has an
uncommonly precise timer.  Coupled with a really good clock and an
accurate time source, it can do sub-microsecond-accurate timekeeping.
