It looks like something is sending a +++ (modem hangup) signal to your
modem. This could be caused by any number of items including the
authenticating procedure. If your isp requires chap authencation then
you have to make sure that your /etc/ppp/options file has the proper
settings applied and the /etc/ppp/chap-secrets/ directory contains the
files and stuff.
