I believe that microsoft originated it as "automatically get /favicon.ico"
later, the html standards people came up with <link rel=...>. some browsers
use one method, some the other, some both, some neither...

my web pages use the <link rel...> form linking to a gif. no favicon
anywhere. feel free to view source[1].
