You're making the assumption that (1)there are managers who care about 
the people who work under them and/or (2)the insurance/legal types have 
noticed this disaster waiting to happen and demanded that something be 
done about it.

In any case, the kinds of dangers that managers ignore every day are 
pretty scary sometimes.  Don't give them too much credit for putting 
the safety of their subordinates ahead of short term gain through 
negligence.
