When I'm not running X, the issue is typically bandwidth.
In my case, I run mutt over SHH from Windows XP (using putty). And I have crappy upload bandwidth.
Still, if
that's not the case for you, you could invoke w3m on an html file that
you concoct to display the image inline. It's a short script that you
then set to be your non-X viewer.
Don't know how to do that, actually.
Remember that cacaview lets you zoom.  Cacaview rocks if you squint. ;-)
