I use a few but my main provider is callwithus (callwithus.com) it is a small shop linux based provider and provides sip and iax interfaces. The have dids also but I do not have a did with them. One nice thing is that they allow you to set your callerid. This is a pay as you go operation. Currently about .01/min but I seleceted a higher quality .015/min rate. I have no complaints and anytime I did have a problem I go an email answer in minutes. Based in NNJ I believe.
