If you're interested in rolling your own solution, voip.ms has some very affordable services. All you need is a supported voip phone, or, if you want to get fancy, an in-home PBX.

I use the latter, running a freePBX instance with a voip.ms trunk. It's 99 cents per DID and 1 cent a minute.
