They are definitely "external" to the main content, though they are connected/linked.



That is, if you have a complete API for creating PDF (through Java,
Perl, etc.) you have the tools for creating bookmarks.

True. If you want open source, there aren't too many options though :(. There's an early Perl attempt (Text::PDF).
