        Just make sure that you are using a version of Ghostscript later 
than 6.0 (current version is 7.0.3), since the 5.x versions of GS generated 
REALLY CRAPPY Postscript.

        Mac OS and Windows :(.

        You can actually incorporate that into your source document using 
something called PDFMarks 
(http://www.pdflib.com/pdfmark/pdfmark_primer.pdf), which is always better 
than "after the fact".
