DCA.NET. No blocked ports, no transfer restrictions, no server type
restrictions. I run www and ftp with their blessing. No PPPoE, using Enet
encapsulation. 1.5Mb/384Kb, 90$ a month. NAT is handled by the very nice
Zyxel gateway, does DHCP, includes  4 port switch. NAT is supported by the
company, offered me help setting it up. I've been dealing with heavy
throughput in both directions 24/7 for about three months, no complaints.
