Albatron has a mini-itx board (KI51PV) that takes an AMD 64.
I'm currently using a bunch of Flex-ATX boards ( smaller than micro-ATX ) and they fit into the phonebook sized NetVista cases. Most of these cards have built-in video/LAN etc. and 2 PCI slots. The parts for 1G Celeron barebone MB/CPU and case/PS is now something like $65 from parts on Ebay. I doubt you'll get anywhere close to those prices with ITX.
