I think its a configuration thing.  It works as expected when embedded in 
konqueror, but not as a stand-alone app.  I can't find a config file that 
controls in in /usr/share/apps/kaffeine, and I deleted 
~/.kde/share/apps/kaffeine

I think there is some other place where the configuration is stored, because 
it remembered the directory in the file browser pane even after I deleted 
~/.kde/share/apps/kaffeine
